Critical access hospital

A small rural hospital with its own Medicare designation — and the same discharge and appeal rules as any other.

What it means

The designation exists to keep hospital services available in rural areas, and it changes how the hospital is paid rather than what a patient is entitled to.

The Medicare rules on discharge notices, the appeal, the qualifying stay for rehabilitation and the observation notice all reach critical access hospitals expressly.

Why it matters

A family in a rural area may be told, or may assume, that a small hospital works differently. On the things that matter here, it does not.

Days spent as an inpatient in one count toward the qualifying stay for rehabilitation.

When you are likely to meet it

  • When a parent is admitted to a small rural hospital.
  • When a transfer to a larger hospital is being discussed.
